# How to Compare Private Health Insurance

By [**Morehealth**](https://www.more-health.co.uk/blog/author/morehealth) on Oct 28, 2026

![\<span id="hs\_cos\_wrapper\_name" class="hs\_cos\_wrapper hs\_cos\_wrapper\_meta\_field hs\_cos\_wrapper\_type\_text" style="" data-hs-cos-general-type="meta\_field" data-hs-cos-type="text" \>How to Compare Private Health Insurance\</span\>](https://www.more-health.co.uk/hubfs/AI-Generated%20Media/Images/Well%20Organized%20Plan%20Health%20Insurance%20Workspace.png)

To compare health insurance properly, line policies up on the same terms before you look at price: the level of outpatient cover, cancer cover, underwriting, hospital list, excess and any options that restrict cover to lower the premium. Two quotes that look similar on price can be very different policies. The cheaper one only makes sense if it still pays for the treatment you would actually want.

That is the short version. The rest of this guide takes each factor in turn, shows you what to look for in the policy documents and explains where comparison sites help and where they fall short.

It is a bigger market than many people assume. LaingBuisson's[21st Health Cover UK Market Report](https://go.laingbuisson.com/health-cover-report-20) found around 8.43 million people in the UK were covered by private medical cover at the end of 2024. More people buying means more policy variations to wade through, which is exactly why a like for like comparison matters.

## **What should you compare when choosing health insurance?**

Compare 7 things side by side: what the policy pays for when you need a diagnosis, how it handles cancer, how your medical history is underwritten, which hospitals and consultants you can use, the excess and how it applies, any options that limit cover to cut the price, and how the premium is likely to change at renewal. Price comes last, not first.

- Level Of Outpatient Cover
- Cancer Cover And Drug Limits
- Underwriting Method
- Hospital List And Consultant Choice
- Excess Amount And How It Applies
- Guided Or 6 Week Options
- Renewal Terms And No Claims Discount

It also helps to know what goes into a quote. When someone contacts us, the first things we ask for are a date of birth, a postcode and a name, because age and location drive much of the price. It is also why we do not show instant prices online. A figure produced without those details is little more than a guess.

## **Start with what you want the policy to do**

Before you request a single quote, decide what you are trying to protect yourself from. A policy is only good value if it pays out for the things that would actually worry you.

Say you are 55, self-employed, and your main concern is waiting months for a knee scan and surgery while you cannot work. Fast diagnosis and inpatient surgery are your priorities. Now picture a 30 year old office worker who mostly wants quick physio for a bad back and access to talking therapy. Same market, very different shape of policy.

Write your priorities down. It sounds basic, but it stops you being steered by whichever quote happens to be cheapest. If the cover is just for you, our guide to[individual health insurance](https://www.more-health.co.uk/individual-health-insurance) explains the options in more detail.

## **Compare the level of cover, not just the headline price**

Most UK policies are built from 2 layers. Core cover pays for inpatient and day patient treatment, such as surgery and a hospital stay. Outpatient cover pays for the consultations, tests and scans that lead to a diagnosis. Some policies include outpatient cover in full, some cap it at a set amount each year and some leave it out altogether.

That difference matters more than people expect. Without outpatient cover, you would typically be diagnosed through the NHS or pay for the diagnosis yourself, then claim for the treatment that follows. For plenty of people the wait for a diagnosis is the part they most want to avoid.

Outpatient limits deserve a closer look too. A limit of £500 or £1,000 a year can sound generous until you add up a specialist consultation, a follow up appointment and an MRI scan. Private fees vary by hospital and region, so ask how far a given limit would realistically stretch where you live.

Then check the extras that are often capped separately. Therapies such as physiotherapy and osteopathy may come with their own limit. Mental health cover is frequently an optional add on, with caps on the number of sessions, the amount spent or the days of inpatient care.

## **How cancer cover differs between policies**

Cancer is one of the main reasons people buy private medical insurance, yet it is also one of the areas where policies differ most. Some pay in full for diagnosis, surgery, chemotherapy, radiotherapy and certain drugs not routinely funded by the NHS. Others cap the amount, restrict drug cover or pass you back to the NHS once NHS treatment is available.

When you compare cancer cover, look at:

- Overall Financial Limits
- Cover For Drugs Not Funded By The NHS
- Monitoring After Treatment Ends
- Palliative And End Of Life Care
- NHS Cash Benefits

If cancer cover is high on your list, it is worth reading our side by side guide to[cancer cover compared](https://www.more-health.co.uk/cancer-cover-compared) before you choose a level of cover.

## **Understand underwriting before you compare quotes**

Underwriting is how the insurer decides what your medical history means for your cover. With moratorium underwriting, anything you have had symptoms, treatment or advice for in the past 5 years is excluded at first. It can become eligible once you have been on the policy for 2 continuous years without symptoms, treatment or advice for it. Full medical underwriting means declaring your history upfront, so you know your exclusions from day one.

This is the part of a comparison that most often goes wrong. Two quotes on different underwriting terms are not like for like, even when everything else matches. Moratorium is usually quicker to set up, but claims can take longer in the early years because the insurer may need to check your records first. Full medical underwriting asks more questions at the start and gives you more certainty later.

If you already hold a policy, there is a third route. Switching on continued terms, often called continued personal medical exclusions, carries your existing underwriting across to a new insurer rather than starting again. Our page on[how health insurance underwriting works](https://www.more-health.co.uk/health-insurance-underwriting) covers each method in more depth.

### **Pre-existing and chronic conditions**

Pre-existing conditions are usually excluded, at least to begin with. Under a moratorium, some may become eligible later in the way described above. Under full medical underwriting, the insurer will tell you which conditions are excluded, and depending on the insurer's terms an exclusion can stay in place for as long as you hold the policy.

Chronic conditions work differently. Private medical insurance is designed for acute conditions, ones likely to respond quickly to treatment. Ongoing management of a chronic condition such as diabetes, asthma or arthritis is not covered. Many policies will pay for the initial diagnosis and for treatment of an acute flare up, but not for the routine check ups and medication that follow. Wording varies, so read this part of each policy closely.

We spend a lot of time on this with people who have a medical history, because the same history can be treated quite differently by different insurers. Some insurers, for example, do not ask you to disclose certain stable chronic conditions when you switch to them, which can affect how a later flare up is handled. Our guide to[chronic conditions and health insurance](https://www.more-health.co.uk/chronic-conditions-explained) explains where the lines usually fall.

## **Hospital lists and guided consultant options**

Every policy comes with a hospital list, the private hospitals and clinics where you can be treated. Broader lists cost more, and lists that include central London hospitals usually cost the most. Before comparing prices, check that the hospitals near your home, or near work, are on each list. A cheaper policy that leaves out your nearest private hospital is not much of a saving.

Most leading insurers also offer a guided option, sold under names such as AXA Health Guided, Aviva Expert Select, Bupa Guided Care and Vitality Consultant Select. Instead of choosing any consultant you like, the insurer offers you a shortlist of approved specialists or hospitals. In return the premium is usually lower, and the insurer will typically settle eligible bills in full.

That last point matters because of shortfalls. If you choose a consultant whose fees are higher than your insurer will pay, you can be left to cover the difference. With a guided option you give up some choice, but you are less exposed to that risk. Our guide to[guided consultant options](https://www.more-health.co.uk/guided-options-explained) compares how each insurer runs theirs.

## **Excess, the 6 week option and other price levers**

The excess is the amount you pay towards a claim before the insurer pays the rest. A higher excess lowers your premium, but how it applies matters just as much as the amount. Some insurers charge it per claim, others once per policy year.

Here is why that matters. Choose a £250 excess that applies per claim, then make 2 unrelated claims in a year, and you pay £500. The same £250 charged once per policy year would cap you at £250.

The 6 week option is another common way to reduce the premium. If the NHS can provide the treatment you need within 6 weeks, you use the NHS. If the wait is longer, your private cover kicks in. Our article on the[6 week option for health insurance](https://www.more-health.co.uk/blog/six-week-healthcare-option) explains who it tends to suit.

It is also worth checking how each insurer's no claims discount works, since a claim can push your premium up at renewal, and whether paying annually rather than monthly brings the price down. For a fuller picture of what moves the price, see our guide to[how much private medical insurance costs](https://www.more-health.co.uk/how-much-does-private-medical-insurance-cost).

## **Health insurance comparison checklist**

Use this as a checklist when you have 2 or more quotes in front of you.

| **What to compare** | **What to check** | **Why it matters** |
| --- | --- | --- |
| Outpatient cover | Full, limited (and the limit) or none | Decides whether your diagnosis can be private |
| Cancer cover | Financial limits, drug cover and any NHS fallback | One of the biggest differences between policies |
| Underwriting | Moratorium, full medical underwriting or continued terms | Shapes what is excluded and how quickly claims are approved |
| Hospital list | Your nearest private hospitals are named | A lower premium can mean a narrower list |
| Consultant choice | Open choice or a guided shortlist | Guided options can lower the premium and the risk of shortfalls |
| Excess | The amount, and whether it is per claim or per year | Changes what you pay each time you claim |
| 6 week option | Included or not | A lower premium in exchange for using the NHS when waits are short |
| Mental health and therapies | Limits on sessions, spend or inpatient days | Often add ons with tight caps |
| Renewal | No claims discount rules | Affects your premium after you claim |

## **How to compare health insurance providers**

There is no single best health insurer for everyone. The main UK providers, including Aviva, AXA Health, Bupa, The Exeter, Vitality, WPA and National Friendly, each build their cover differently. The right one for you depends on how a policy handles the things you care about and how it treats your medical history, not on the brand name.

Beyond the policy wording, look at how each insurer works in practice. How do you get a claim authorised, by phone or through an app? Is there a digital GP service? Will the insurer settle bills directly with the hospital? Wellbeing rewards and gym discounts can be a nice extra, but they should not be the reason you pick a policy. You can[compare private health insurance providers](https://www.more-health.co.uk/health-insurance-providers) on our dedicated page.

## **Comparison sites, going direct or using a broker**

Comparison sites are useful for a quick price benchmark if you already know what you want. Going direct to an insurer gives you that insurer's view and nobody else's. An independent broker compares policies across the market and explains the differences in cover, which matters most if you have a medical history, are switching, or are buying for a business.

Comparison sites only show insurers on their panel, and the cheapest result at the top of the page may also be the most restricted policy. So if you use one, look behind the price at the outpatient limit, hospital list and underwriting before you buy.

If you use a broker, it is reasonable to ask how they are paid. We are paid commission by the insurer when you take out a policy. It is not added to your premium, and our advice is free to you. Whoever you use, you can check that a firm is authorised on the[FCA Financial Services Register](https://register.fca.org.uk/). Morehealth's FCA number is 815733.

## **Comparing health insurance for a business**

The same principles apply when you compare cover for a team, with a few extras. Group schemes can offer underwriting options that are not available on individual policies, such as medical history disregarded, depending on the insurer and the size of the group. You will also want to think about who is covered, whether employees can add family members and how the scheme will be managed at renewal.

There is a tax angle too. When an employer pays for private medical insurance, it is generally a benefit that needs reporting to HMRC, and National Insurance may be due on it.[GOV.UK's guidance on medical treatment and insurance benefits](https://www.gov.uk/expenses-and-benefits-medical-treatment) sets out the current rules, and your accountant can confirm how they apply to you. For more on setting up cover for a team, see our page on[business health insurance](https://www.more-health.co.uk/business-health-insurance).

## **Comparing at renewal and before you switch**

Renewal is the natural time to compare. Most of the policy reviews we carry out are prompted by a renewal letter, and in our experience those tend to cluster in spring and autumn. A review can show potential savings, or confirm that your current cover is still the right fit.

Switching needs care, though. Imagine you had physio for a back problem 18 months ago under your current policy. Move to a new insurer on a fresh moratorium and that back problem would likely be excluded again. Switching on continued terms, or staying where you are and asking your insurer to review the price, may suit you better.

Whatever you decide, do not cancel your existing policy until the new one is confirmed and in place. Our guide to[switching health insurance providers](https://www.more-health.co.uk/blog/switching-health-insurance-companies-guide) walks through the process step by step.

## Frequently asked questions

What is the best site to compare health insurance?

There is no single best site. Comparison sites give you a quick price snapshot from the insurers on their panel, which is a useful starting point. An independent, whole of market broker can compare those policies in more depth and explain the exclusions. Many people do both: benchmark online, then check the detail with an adviser before they buy.

Who is the best health insurance provider in the UK?

It depends on what you need. One insurer may have the strongest cancer cover for your budget, while another offers the hospital list that suits where you live. The best provider for you is the one whose policy covers your priorities on terms that work with your medical history.

What is the best way to compare health insurance plans?

Decide your must haves first, then get quotes on the same terms: the same excess, underwriting method, hospital list and level of outpatient cover. Only once those match is a price comparison meaningful. If the terms differ, you are comparing different products rather than different prices.

Is it cheaper to use a health insurance broker?

Using a broker does not add to your premium. We are paid commission by the insurer, not by you, and our advice is free. A broker may be able to find potential savings by adjusting the excess, hospital list or underwriting, but whether you save depends on your circumstances and the cover you need.

Can I compare health insurance if I have a pre-existing condition?

Yes, and comparing is arguably more important in that case. Insurers can treat the same condition differently depending on the underwriting method and their own rules. Comparing how each one would handle your history, not just the price, gives you a clearer picture of what you would actually be covered for.

How often should I compare my health insurance?

Each renewal is a sensible time, ideally a few weeks before your renewal date so you have time to weigh up the options. It is also worth a review after a big life change, such as retiring, becoming self-employed or adding a partner or children to your cover.
